You, the person who needs care, are always the most important person. Then:

  • health professionals
  • your family and friends
  • your local council, NHS and social worker
  • people and organisations who are paid to support you
  • your local community: clubs, groups, leisure centre, employer…etc

It can be a lot to think about!

Troika will:

  • Listen to you to find out about your likes, dislikes, plans and dreams
  • Communicate with your council and NHS
  • Help you to find people and organisations who can support you
  • Manage the money and all of the administration

Once everything is set up, we will keep in touch with you. We will ask you what is going well, and what needs to change. If you want to change something, we will work together to make this happen.

Remember, this is a team effort. We do not take over. You are involved, every step of the way.

If you cannot take part in a decision about your care, we follow the Mental Capacity Act. This means we:

  1. Assume that you are able to make a particular decision unless an assessment has been done, and it says you cannot.
  2. Support you to make decisions, by giving you information in ways you can understand.
  3. Respect that you have the right to make unwise decisions, if you understand the risks and the decision you are making.
  4. Make decisions in your best interests,  if you are assessed as not being able to make the decision yourself. There are laws we follow about how this is done.
  5. When we are involved in a Best Interest decision, we must always go for the option which gives you the most freedom. This is sometimes called the “Least Restrictive Option.”
